1. Full name
2. IACRA FTN
3. Pilot certificate (front and back)
4. Photo ID (that you will be presenting on the Zoom call)
5. Medical certificate
6. Email and phone number
7. Evidence of completion of the requirements (SOE sheet, copy of your electronic logbook)
How to complete IACRA:
IACRA Application
Log in to IACRA as an Applicant and select “Start New Application” at the top of the console.
Select “Administrative Action” as the Type of Application and then select “Remove Limitation without a Practical Test” from the next section.
From there, a new section will expand and the limitation will most likely be automatically selected.
Then click “Start Application”.
On this Application Process page, it’s all the normal stuff you’re used to. Confirming or updating your Personal Information, Residential Address, Certificate information, etc.
RESTRICTED ATP: Include all flight time.
SOE ONLY: Pilot Time (Section III) ... ONLY PUT 25 hours under flight time. Do not include any other flight time.
Review and submit your application when everything is in order and you’re ready for the next step.

Supporting Documents: Letters from PIC(s) or an Airline Form
The regulations say the “pilot in command who observed the flight attests in writing to each flight” so you must show evidence of this:
If you didn’t fly 25 hours with one single PIC, prepare multiple letters as appropriate. The total hours combined can be more than 25 but a minimum of 25 must be present. If your airline is no longer in business, contact the PICs you flew with. This could be 25 different letters. Or, use a general form and just have the PIC's sign. Docusign does count for signatures, or emailing them and having them sign it and send it back works as well. Here are 2 examples:

Logbook Excerpt
While the letters themselves could possibly be all you need, it’s best to provide an excerpt from your logbook highlighting the specific flights you’re referencing.
Foreflight and Log Ten Pro are great, but you need to show a copy of your logbook instead of the "summaries" section. The DPE needs to see the individual flights.
ATP Restrictions Removal

You have an SOE (Supervised Operating Experience) restrictions on a TYPE RATING if you have the following limitation (substitute CL-65 for any aircraft requiring a type rating):
"The CL-65 is subject to pilot in command limitation(s)"
You have a Restricted ATP if you have the following limitation:
"Restricted in accordance with 14 CFR 61.167"
"Holder does not meet the pilot in command experience requirements of ICAO"
To remove an SOE restriction, it requires ALL of the following to be complied with:
(g) The limitation described under paragraph (f)(2) of this section may be removed from the pilot certificate if the applicant complies with the following—
(1) Performs 25 hours of flight time in an aircraft of the appropriate category, class (if a class rating is required), and type for which the limitation applies under the direct observation of the pilot in command who holds a category, class (if a class rating is required), and type rating, without limitations, for the aircraft;
(2) Logs each flight and the pilot in command who observed the flight attests in writing to each flight;
(3) Obtains the flight time while performing the duties of pilot in command; and
(4) Presents evidence of the supervised operating experience to any Examiner or Flight Standards office to have the limitation removed.
I get a lot of questions from people requesting SOE removal from an old airline that is no longer in business. Your logbook showing the 25 hours does not meet the regulatory requirements. A lot of airlines have a blank SOE sheet that they give their pilots to be filled out. However, this is not the only way of doing it. A company letter with the captain's signature attesting to the flight is also acceptable. I have included a form I created here:
